Hmm, good question! I always assumed they did it manually to ensure the links didn't violate their site's TOS but I could be wrong.

The best directories like DMOZ, or Yahoo approve the submissions manually.
However there are directories that do it automatically.

I am a directory owner. I approve manually and I believe every directory owner does the same.
In most cases you will find that the directory website(s) will ping the given site to see if you have provided a backlink to the directory; and if so they will automatically accept you.
I personally dont recommend this as it is one of the very reasons search engines have started to dis-credit directory backlinks in my opinion.
There are ofcourse many large scale directories and most all of them do manual approval; hence providing a higher quality backlink to your site that actually gets credit, provided that its placed with in a niche area that is.

I have 3 directories. I approve links manually twice a week.
It is very pain to approve links becoz so many link submitters are spamming with adding of keyword stuff in description.
