I had a bout with something last year that we won't get into, but it caused nearly every site to go offline for a month. Most of my domains are 0/10 or 1/10, but there were a few (and individual pages) that had 3/10 and 4/10.
That was July of 2009, now it's almost February 2010 and the high rank sites JUST TODAY got a 1/10 back.
Other bad things that can happen:
- Linking to "shady neighbors" - exchanges with spammy link pages has cost me (and you too probably) PR.
- Poor SEO and coding. Misunderstandings had me linking my logo to myself, but using the full URL instead of "/" including the back-links to myself on those domains.
We all know a low/no PR site can kick ass just as well as a corporate site, but more PR means more PoweR. It's said that a link is a link, but if experience is a teacher, that lesson is not true. You can get PR from links TO other people too, but that's a whole other story.
