I have seen many sites which have there logo visible in the address bar.
How and where can I add the icon, which will be visible in the address bar? Is there any size limit?

This icon you are talking about is called a Favicon, here is a link to a free Favicon generator. As for the size limit... I am not sure if there is a required sized for Favicon.

Usually favicons are 16x16 or 32x32. The file type favicon generators create is an .ico. It's one of those things that gets easier the more you do it.

You can make favicon with using photoshop and can keep it on the site using javascript.

I found a really simple icon maker that can be downloaded from http://www.icon-maker.com/. Its a 15 day trial before buy. The size should be 16x16.

There's also numerous free icons to be used to be found on the net, try http://www.clickfire.com/content/graphics/favicons/ and http://www.ixfix.com/.
